How to safely stop finasteride 1 mg

Patients consider stopping finasteride for a range of reasons including side effects, partner pregnancy planning, satisfaction with current density, or switching to dutasteride.

Common reasons patients consider stopping

  • New or worsening side effects (sexual, mood, or other)
  • Partner is pregnant or planning pregnancy (handling precautions)
  • Stable density achieved and questioning whether to continue

What the MHRA says

MHRA Drug Safety Updates 29 April 2024 and 11 May 2026 acknowledge that psychiatric (depression, suicidal thoughts) and sexual side effects (reduced libido, erectile dysfunction, ejaculation disorders) have been reported to persist after stopping in a small number of cases.

The pharmacology of stopping

Pharmacologically, finasteride is cleared within days (~6-hour half-life). Serum DHT returns to baseline within approximately 2 weeks. Hair benefit accrued over years of treatment is typically lost over 6 to 12 months without ongoing therapy, but the exact regression varies between individuals.

What to expect for your hair

Over 6 to 12 months, hair density typically returns toward the pre-treatment trajectory. The benefit accumulated during years of treatment is real but not retained without the medication.

How to stop, step by step

  1. Speak to your prescriber before stopping

    Confirm the reason for stopping, document any current side effects (especially mood or sexual), and ask whether a switch (e.g. to topical only) might address the concern without complete cessation.

  2. Decide between taper and cold-turkey

    Finasteride does not require a taper pharmacologically (no physiological dependence). Some patients prefer to halve the dose for 2 to 4 weeks for psychological transition; others stop in one step. Both are clinically reasonable.

  3. Continue topical minoxidil if appropriate

    If you are stopping finasteride but want to retain density, continuing topical minoxidil under clinician guidance can be a reasonable bridging approach.

  4. Monitor for symptom resolution

    Most side effects resolve within weeks of stopping. The MHRA DSUs specifically recognise that in a small number of cases sexual or psychiatric symptoms may persist; if this occurs, contact us promptly and report via the MHRA Yellow Card scheme.

  5. Review hair at 3, 6 and 12 months

    Document density with consistent lighting and angle photography so the regression trajectory is visible. This makes any decision to restart, switch or transition to surgery clinically informed.
