Safe discontinuation

How to safely stop oral minoxidil (low dose)

Oral minoxidil is prescribed off-label for hair loss in the UK and has a clear set of reasons patients consider stopping. The cardiovascular profile means there is a particular care pattern at discontinuation.

Common reasons patients consider stopping

  • Facial or body hypertrichosis (unwanted hair elsewhere)
  • Ankle oedema or fluid retention
  • Postural symptoms or palpitations
  • Wanting to switch back to topical-only

The pharmacology of stopping

Oral minoxidil for hair loss is used at much lower doses (0.625 to 5 mg) than the antihypertensive dose. Cardiovascular adverse effects, when present, are dose-related and typically resolve on stopping.

Half-life
Approximately 4 hours.
After stopping
Pharmacological effect is gone within days. Hypertrichosis takes weeks to months to resolve as the new hairs that were stimulated complete their cycle.
Hair timeline
Scalp hair regression is similar to topical: 3 to 6 months.

What to expect for your hair

Scalp hair gained through anagen prolongation is lost over the next 3 to 6 months. Body and facial hair stimulated by treatment typically settles over weeks to months as those hairs complete their growth cycle.

How to stop, step by step

  1. Speak to your prescriber

Discuss the specific symptom driving the decision; a dose reduction may resolve the issue without complete cessation, particularly for mild oedema or hypertrichosis.

  2. Consider whether the symptom warrants urgent review

Palpitations, breathlessness or new postural symptoms warrant prompt clinical review before just stopping.

  3. No taper required pharmacologically

For most patients on low doses for hair loss, abrupt discontinuation is appropriate. Patients who started from very low doses (0.625 mg) often need no transition.

  4. Expect scalp hair regression over 3 to 6 months

This is the reverse of the response seen on starting.

Bloods to help the decision

A pre- and post-stopping bloodwork plan helps you and your prescriber see what is actually happening, rather than rely on impressions. Our sister diagnostic service, WMG Health, offers panels designed around the question you are asking:

  • Hormone baseline before stopping: total testosterone, free testosterone, SHBG, oestradiol, prolactin. The Hormone Specialist panel from £299, or a custom panel if you want something more targeted (from £275).
  • Hair density and nutritional baseline: ferritin, thyroid, vitamin D, B12. Hair Loss Essentials from £249.
  • Custom workup for your specific question: if you have side effects you want investigated (mood, sleep, energy, libido), a clinician-designed custom panel is usually the best route. Quote within 1 working day.

Same Westminster Medical Group, single clinical record, results visible to your WMG Meds prescriber.

Thinking about stopping?

Do it with a clinician, not on your own

Stopping hair loss treatment has predictable consequences that are much easier to manage when they are planned for. A GMC-registered doctor can talk through the timeline, what to expect, and whether a different regimen would suit you better than stopping altogether.
