Does ketoconazole shampoo help hair loss?

Short answer

The evidence is limited. Ketoconazole is an antifungal shampoo used for dandruff and seborrhoeic dermatitis, and a few small studies suggest it may help pattern hair loss, but a 2020 systematic review concluded that randomised controlled trials are still needed. It is not licensed for hair loss in the UK, and WMG Meds does not currently prescribe it.

Key facts

  • Ketoconazole shampoo is an antifungal used for dandruff and seborrhoeic dermatitis.
  • Up to 2% can be sold from pharmacies for those uses in packs of up to 120 ml (BNF).
  • A 2020 review found five small human studies and said randomised trials are needed.
  • It is not licensed for hair loss, and WMG Meds does not currently prescribe it.

What ketoconazole shampoo is for

Ketoconazole is an antifungal medicine. As a shampoo, the BNF lists it for treating and preventing seborrhoeic dermatitis (a common flaky, itchy skin condition) and dandruff, and for a skin infection called pityriasis versicolor. The NHS dandruff page suggests looking for a shampoo containing ketoconazole, coal tar or salicylic acid, and says a pharmacist can tell you how to use it.

In the UK, the BNF says ketoconazole shampoo of up to 2% strength can be sold to the public for dandruff and seborrhoeic dermatitis of the scalp, in packs of up to 120 ml labelled for use no more than once every 3 days. Outside those limits it needs a prescription. Hair loss is not one of its licensed uses.

What the studies show for hair loss

The idea is that yeasts on the scalp and the inflammation around hair follicles might play a part in androgenetic alopecia, and that an antifungal with some anti-inflammatory activity could help.

Piérard-Franchimont and colleagues, 1998: compared 2% ketoconazole shampoo with an unmedicated shampoo, used with or without 2% minoxidil. They reported improvements in hair density, hair size and the proportion of growing hairs with ketoconazole, and said the clinical significance "awaits further controlled study in a larger group of subjects".

Fields and colleagues, 2020 (systematic review): found five human studies with a total of 318 participants, plus two animal studies. Human studies reported increased hair shaft thickness and some clinical improvement. The authors called ketoconazole "promising" but concluded that "randomized controlled trials are needed".

In short: small, older studies, and not enough to recommend it as a hair loss treatment on its own.

Where it may fit

If you have dandruff or seborrhoeic dermatitis as well as thinning, treating the scalp condition makes sense in its own right, and a pharmacist can advise. The NHS says to see a GP if dandruff has not improved after using an anti-dandruff shampoo for at least 4 weeks, or if your scalp is very itchy, red or swollen.

If you use a licensed minoxidil product, note that the Regaine SmPC lists use alongside "other topical medical preparations" on the scalp as a reason not to use it. Check with your pharmacist or prescriber before combining ketoconazole shampoo with any scalp treatment.

WMG Meds and ketoconazole

WMG Meds does not currently prescribe ketoconazole shampoo. The medicines our doctors can consider after assessment are listed on our products page, and how treatments work explains the evidence behind them.

If you would like advice on whether medical treatment is suitable for your hair loss, the free online assessment asks about everything you currently use, including shampoos and supplements. A sore, inflamed or scarring scalp needs a GP or dermatologist rather than an online service: see scarring alopecia.

Related questions

Is ketoconazole shampoo prescription-only in the UK?

Not always. The BNF says up to 2% can be sold to the public for dandruff and seborrhoeic dermatitis in packs of up to 120 ml, labelled for use no more than once every 3 days. Other uses and pack sizes need a prescription.

How often is ketoconazole shampoo used for dandruff?

For seborrhoeic dermatitis and dandruff, the BNF gives twice weekly for 2 to 4 weeks to treat it, then every 1 to 2 weeks to prevent it coming back, leaving it on for 3 to 5 minutes before rinsing. Follow the pack or your pharmacist's directions.

Can ketoconazole shampoo replace finasteride or minoxidil?

There is not enough evidence to say it can. The studies are small and the 2020 review called for proper randomised trials.

Can I use ketoconazole shampoo with minoxidil?

Ask your pharmacist or prescriber first. The Regaine SmPC says not to use it alongside other topical medical preparations on the scalp.

Does WMG Meds prescribe ketoconazole?

No, not currently. Our doctors can consider finasteride, dutasteride, oral minoxidil and compounded topicals after assessment.
